By agreeing below, I acknowledge and fully understand that participation in Path Academy FC programs—including but not limited to tryouts, practices, games, clinics, and training sessions—carries inherent risks of injury, illness, or accident, including serious bodily harm, death or property damage. I voluntarily assume all risks and agree to release and hold harmless Path Academy FC, its staff, coaches, volunteers, affiliates, and partner organizations from any and all liability, claims, demands, or causes of action arising out of or related to participation in any club activity. I also authorize Path Academy FC staff to obtain emergency medical care if necessary and understand that I am responsible for primary insurance coverage. I affirm that the participant is in good health and physically fit to take part in club activities. Furthermore, I grant full permission to Path Academy FC to use photographs, video, and audio recordings of the participant taken during any club activity for promotional, educational, and informational purposes across all media platforms. This permission is granted without expectation of compensation and applies to use by the club and its approved partners. I have read and understand this waiver and agree to its terms on behalf of myself and/or my child.
